Altar Servers – St Peter Catholic Church

Parental Permission & Media Release

I am the parent or legal guardian of the child named above. I have read this application, and I agree to the following so that my child may serve at the altar of St. Peter Catholic Church.

I. Permission to Serve: I grant permission for my child to participate in the Altar Server Ministry of St. Peter Catholic Church, including serving at weekend Masses, holy days, funerals, and other liturgies as scheduled, and attending required training (classes, instructional materials, or shadowing experienced servers).

II. Duties and Conduct: I understand that altar servers assist the priest and deacons with reverence at the altar. My child is expected to arrive on time, dress appropriately, behave with dignity in the sanctuary, and notify the coordinator if unable to serve an assigned Mass. I will support my child in these responsibilities.

III. Photography, Video, and Livestream: I understand that the 9:00 a.m. Sunday Mass at St. Peter Catholic Church is regularly livestreamed on the internet, and that persons in the sanctuary — including altar servers — may be visible and audible in that livestream and in any recording of it. Appearance in the Mass livestream is incidental to serving that Mass. I understand and consent to my child's appearance in the livestream and recordings of Mass, including the 9:00 a.m. Sunday Mass.

Separately, the parish may wish to use photographs or video of altar servers in the bulletin, on the parish website, on social media, or in other parish communications.
