You are receiving this form because our records indicate that you are either currently serving as, or have previously been considered or trained to serve as an Extraordinary Minister of Holy Communion (EMHC). As we review and renew our ministry rosters, please complete the form below to help us understand your current status and discernment.

As part of this renewal process, Father Medina is required to attest to the following for each candidate:

  1. That each candidate is personally known to him;
  2. That each candidate continues to be an exemplary member of the Catholic community;
  3. That each candidate demonstrates a solid understanding of the vocation of a baptized and confirmed Christian;
  4. That each candidate has a proper understanding of the theology of the Eucharist;
  5. That each candidate is striving to live a holy life;
  6. That each candidate is free of any impediments and has freely consented to serve; and
  7. That she/he is willing to assume responsibility for the ongoing spiritual development of each EMHC.

  3. Partake in regular formation: parish missions, parish Bible study programs, diocesan and parish programs based on the Eucharist, parish retreat, virtual workshops on EMHC ministry from Liturgy Training Publications, and Office of Liturgy ongoing formation of EMHC minister classes.
